If you are at that step, an endocrinologist is required. If tsh is too high and ft3,ft4 are too low, thats manifest hypothyroidism (requiring L-Thyroxine). If tsh is too high, but ft3, ft4 are normal, thats latent hypothyroidism, meaning the hormones produced by the thyroid glands are still sufficient and that would require checking on but no meds. Also get you hemoglobin checked while your at it, tiredness, feeling cold and also being exhausted easily can be signs of anemia. If those are normal, you don't have hypothyroidism (which it sounds like is the thing you suspect?).
